Overview

Chloromycetin is one of the earliest natural antimicrobial agents developed in the late 1940s. Over time, many bacteria have developed resistance to it. Due to its significant side effects, its use today is limited. It is primarily prescribed for infections such as meningitis, rickettsial diseases, salmonellosis, and anaerobic bacterial infections. The drug works by inhibiting bacterial protein synthesis at the ribosomal level, exerting mostly bacteriostatic effects, though it can act bactericidally at high doses against pathogens like pneumococci, meningococci, and Haemophilus influenzae. Chloromycetin is commonly applied locally for bacterial eye infections and occasionally used systemically for severe infections such as bacterial meningitis, brain abscesses, generalized salmonellosis, typhoid fever, rickettsiosis (including Ku-fever, Rocky Mountain spotted fever, typhus), intra-abdominal and pelvic infections, gas gangrene, and plague as a secondary treatment.

Dosage and Instructions

Only use Chloromycetin if no safer or more effective alternatives exist. Follow your prescription carefully without missing doses, taking them at evenly spaced intervals. Avoid repeated courses of the same medication. Tablets should be taken with a full glass of water. For local liquid applications, strictly adhere to usage instructions provided by your healthcare professional.

Potential Side Effects

Though rare, Chloromycetin can cause severe blood disorders such as bone marrow hypoplasia and aplastic anemia, necessitating regular blood monitoring during treatment. Use of antibiotics may disrupt normal gut flora, potentially causing gastrointestinal issues ranging from mild diarrhea to life-threatening colitis. In newborns, a condition called gray baby syndrome may occur, characterized by hypothermia, cyanosis, muscle weakness, and circulatory failure, which is often fatal.

Drug Interactions

Chloromycetin can prolong the elimination of oral anti-diabetic medications and may produce false-positive glucose test results in diabetic patients. It should not be combined with erythromycin or lincosamide antibiotics. Additionally, it may reduce the effectiveness of drugs containing iron, folic acid, or vitamin B12.

Missed Dose

If a dose is missed, do not double up. If it is near the time for the next dose, simply skip the missed one and continue the regular schedule.

Overdose

Symptoms of overdose can include easy bruising or bleeding, persistent sore throat, fever, unusual tiredness, headache, mild depression, mental confusion, rashes, and swelling under the skin (angioedema). If overdose is suspected, especially accompanied by watery or bloody diarrhea, stomach cramps, or fever, seek immediate medical attention.

Storage

Store Chloromycetin at room temperature (15-25°C or 59-77°F), protected from light and moisture, and out of reach of children and pets. If refrigerated, do not freeze the medication.
