Generic Macrobid ( nitrofurantoin monohydrate )

Overview of Macrobid
Macrobid is a broad-spectrum antibiotic that works by disrupting bacterial cell wall formation, DNA and RNA synthesis, protein production, and aerobic metabolism. It is commonly prescribed to treat urinary tract infections caused by bacteria such as E. coli, Enterobacter, Staphylococcus aureus, Enterococcus, and Klebsiella. Conditions treated include pyelitis, pyelonephritis, cystitis, and urethritis. Additionally, Macrobid is used to prevent infections during urological procedures like surgeries, cystoscopy, and catheterization. This medication is a sustained-release macrocrystalline form of nitrofurantoin, with absorption affected by crystal size and food intake.
Dosage and Administration
The typical dosage is 100 mg of Macrobid every 12 hours. Taking the medication with food or milk enhances absorption and helps prevent stomach discomfort. The usual treatment duration lasts seven days. Always follow your healthcare provider's instructions carefully and do not start or stop taking Macrobid without medical advice.
Precautions
- Inform your doctor if you have kidney disease, anemia, diabetes, electrolyte imbalance, glucose-6-phosphate dehydrogenase (G6PD) deficiency, vitamin B deficiency, or any chronic illness.
- Macrobid may interfere with urine glucose test results.
- Diarrhea is a possible side effect; if you experience watery or bloody stools, contact your doctor immediately.
- Avoid antidiarrheal medicines unless prescribed by your healthcare provider, as diarrhea may indicate a new infection.
Contraindications
- Macrobid should not be used by patients with kidney or liver failure, history of jaundice, G6PD deficiency, or acute porphyria.
- It is contraindicated during pregnancy and in children under 12 years of age.
- Breastfeeding is not recommended while taking Macrobid.
Possible Side Effects
- Common side effects include headache, dizziness, drowsiness, allergic reactions like rash or itching, nausea, and vomiting.
- Less common effects may include diarrhea, loss of appetite, pseudomembranous colitis, and abdominal pain.
Drug Interactions
High doses of probenecid (Benemid) or sulfinpyrazone (Anturane) can reduce the kidney's ability to eliminate Macrobid, leading to increased blood levels and risk of toxicity. The antibacterial effect of Macrobid may be diminished by nalidixic acid, and absorption can be reduced by antacids containing magnesium trisilicate. Co-administration with quinolone antibiotics such as norfloxacin (Noroxin) can decrease their effectiveness.
Missed Dose
If you forget to take a dose, take it as soon as you remember unless it is almost time for your next dose. In that case, skip the missed dose and continue with your regular schedule. Do not double the dose to compensate.
Overdose
Overdose symptoms may include nausea and vomiting. If you suspect an overdose, seek emergency medical care immediately.
Storage
Store Macrobid at room temperature in a tightly closed container, away from moisture and humidity.
